The MFT continuing education certificates you received for the 2006 Spring Conference and writing workshop with Doug Flemons held on April 20, 21, 2006 were incorrectly dated 2004. Please cross out 2004 and write in 2006.
The Minnesota Board of Marriage and Family Therapy is aware of this error.
Correction!
The CEU certificate for the Fall Conference Keynote on Thursday, November 8th, 2007 was printed as .75 credits and it should have said 2.0 credits. Please make this correction on your certificate.
